The Statutory Accident Benefits Form
The only way to receive these benefits is to report your car accident and damages to your property. If you’re injured, you must also report your injuries. The Statutory Accident Benefits Form is where you report your accident and related damages and injuries. Without completing this form, you cannot receive the benefits you are due.
